Rehabilitation Services

Our licensed staff members offer quality inpatient and outpatient rehabilitation. We work closely with patients and their families to set and meet rehabilitation goals through evaluation, treatment, education, patient and family teaching, provision of specialized exercise programs and follow-up care.

Wound Care & IV Therapy

Post-surgical patients or patients with lesions, will be provided a program that focuses on pain management needs to help, heal, and comfort them. We also provide IV Therapy to aid in patients recovery.

Notice of Data Breach

WebTPA Employer Services, LLC (“WebTPA”) detected a data security  incident impacting certain systems on their network. WebTPA acts as an administrative services provider to certain benefit plans whose information was impacted in this incident.

On December 28, 2023, WebTPA detected evidence of suspicious activity on their network that prompted an investigation. Measures were taken to mitigate the threat and further secure their network. They also launched an investigation with the support of cybersecurity experts and notified federal law enforcement.

The investigation concluded that the unauthorized actor may have accessed and/or obtained personal information between April 18 and April 23, 2023. In the course of the investigation, WebTPA determined that your information may have been impacted in this incident. In May of 2024, WebTPA mailed notices indicating your name and date of birth may have been impacted.

JCMH received confirmation from WebTPA on September 30, 2024 that social security numbers may have also been impacted but were referenced as “member numbers” in the original discovery documents and, therefore, not included in the original notification.

WebTPA deployed additional security measures and tools with the guidance of third-party cybersecurity experts to further strengthen the security of their network. JCMH is working diligently to mitigate sensitive information from eligibility data files where appropriate.

WebTPA was not aware of any misuse of your information as a result of this incident. Your financial information, such as credit card numbers, was not involved in this incident. It is always advisable to remain vigilant against attempts at identity theft or fraud, which includes carefully reviewing credit reports and Explanations of Benefits (“EOBs”) from your benefit plans for suspicious activity. If you identify suspicious activity, you should contact the entity that maintains the information on your behalf.
